Covid vaccination status requests

We are understandably receiving lots of enquiries from patients regarding how the prove their covid vaccination status.

A number of the online apps (Patient Access, NHS App, MYGP etc) give patients the ability to view vaccines that have been given, including covid vaccinations.

If you do not have access to these, you can request a letter form the NHS providing yur vaccination status by calling 119 from the 17th May.

We are pleased to be able to offer evening and weekend appointments by working in collaboration with other GP practices in the area.

This does mean that if you want to be seen on a weekday evening or weekend, your appointment may be held at a different GP practice to your registered practice and with a different GP or nurse than you usually see.

The other practices we are working with, and where you may be offered an appointment are:

  • New Longton Surgery
  • St Mary’s Health Centre
  • St Filans Medical Centre
  • Dr Yerra – Gutteridge Medical Centre
  • Dr Shahid – Gutteridge Medical Centre
  • Briarwood Medical Centre
  • Lane Ends Medical Centre
  • North Preston Medical Group
  • Park Medical Practice

Your medical record will be available to the GP or nurse that sees you to ensure that they have the information they need to give you the best possible care. Notes of your appointment will be sent back to us to ensure that your record is updated.

In order to book an evening or weekend appointment, you must call us at the practice as these appointments are not currently available online.

However if you need to book an evening or weekend appointment and your GP surgery is shut, please call NHS 111 and they will be able to advise you further.

Please note that once hub availability is full you may be directed to an alternative service.

From 6:30pm - 8:00pm midweek and at the times stated above at weekends you will be transferred directly to the hub site by calling us on our usual practice telephone number.

(bank holiday opening times may vary).
