Appointments System

Routine appointments to see a doctor or nurse can be made by telephoning the surgery on 01772 972227 during normal surgery hours, in person or online.

Appointments can be booked up to two weeks in advance. These appointments are booked at intervals of 15 minutes. Every effort is made to keep to these times. However, we are sure you will understand that there will be occasions when this overruns.

Please make a separate appointment if more than one member of the family is to be seen.

Same Day Appointments

For consultations that cannot wait for a routine appointment, we will endeavour to give you an appointment on the same day. You may not necessarily see the doctor of your choice.

If all the appointments for the day are booked up, the receptionist will ask you to give a brief outline of your problem and pass it to the on-call doctor who will decide to see you as an emergency or if you need to book a subsequent routine appointment.

Please help us and other patients needing emergency attention by not requesting repeat certificates, repeat prescriptions or referrals and long standing medical problems in the same day surgeries.

Telephone Consultation

We offer telephone appointments aswell as face to face appointments. The care co-ordinator will advise you on the most approproate appointment for you.

Guide on how to take a useful photograph to aid diagnosis

Although photographing sometimes seems simple, there are a few rules of thumb that will make it much easier for your doctor to be able to make a diagnosis.

Adequate lighting

Please make sure the area is well lit. Not flooded with light, but enough light to make the area (for example, a mole on your arm) clear to see, and stand out from the surrounding skin.
Use a plain background

A light coloured sheet or a light coloured wall is ideal.

Some sort of scale

It is important to be able to tell the size of lesion, so place a tape measure or ruler alongside it. Alternatively, something like a 5p or 10p piece coin alongside will be very helpful.

Indication of location In some photographs, it can be very difficult to tell what part of the body the photograph is of. Please make this clear, either in the photograph, or in accompanying message. The side of the body (right or left) is also important. Sometimes you need to take several photos, one from a distance to show location, and closer ones to show detail.

Indication of orientation It can sometimes be difficult to tell which way up a photograph is. Please try to make it clear on the photograph which way is up!

Identification (name, date of birth, and date of photo) We need the details of who the photograph is of. We will need both name and date of birth. This is because some photos we are sent are of a relative (or even a neighbour!). We need to make absolutely sure the photos go in the right set of notes. Each photo should include these details on it. I suggest you write these details on a piece of paper and include it in the photograph. These can then be emailed into our secure email address: gpccg.parkview@nhs.net

Make sure it is in focus! You would be surprised at the number of images we get sent that are blurry. Do your best to make the photos clear and sharp.

Be aware that by sending photographs to your doctor, you are providing your consent for us to receive and store the images and that you are aware they form part of your medical record, and will be filed within the record for this reason. They also might have to be sent on to a specialist for further advice (this is another reason for needing the patient identification details requested above).

Due to the sensitive nature of some photographs, we are not able to accept images of intimate areas/genitalia or children.

 

Home Visits

If possible please try to telephone reception before 10:30am if you require a home visit.

A doctor may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.

House visits are only available for patients who are housebound because of illness or disability.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the surgery.

Extended Access

 

We are pleased to be able to offer evening and weekend appointments by working in collaboration with other GP practices in the area. 

This does mean that if you want to be seen on a weekday evening or weekend, your appointment may be held at a different GP practice to your registered practice and with a different GP or nurse than you usually see.

The other practices we are working with, and where you may be offered an appointment are:

Your medical record will be available to the GP or nurse that sees you to ensure that they have the information they need to give you the best possible care. Notes of your appointment will be sent back to us to ensure that your record is updated. 

In order to book an evening or weekend appointment, you must call us at the practice as these appointments are not currently available online.

However if you need to book an evening or weekend appointment and your GP surgery is shut, please call NHS 111 and they will be able to advise you further.

Please note that once hub availability is full you may be directed to an alternative service.

From 18:30 - 20:00 midweek and at the times stated above at weekends you will be transferred directly to the hub site by calling us on our usual practice telephone number (bank holiday opening times may vary).

Cancel an Appointment

It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters maybe removed from the list.

You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance. (excluding weekends and public holidays). You can also text to cancel your appointment.

Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.

Please remember to update your contact details with us when you change address, telephone numbers and email address.
