I currently undertake structured medication reviews at the surgery to help improve patient safety and outcomes through a person-centred approach. I currently help the patient experience by reviewing medications, discussing any medication side-effects, managing long-term conditions, actioning any medication changes following a discharge from hospital/attending clinical appointments and following up on any appointments made with a GP. I also work as part of the hypertension team at the surgery to help identify and manage any high blood pressure concerns. As with all clinical staff I have both telephone appointments and appointments in a private consultation room depending on preference.
